To send a sample for titer testing at CAVIDS

1. Get a half ml of separated serum from the pet (no blood cells)

2. Print and complete the serum submission form (see separate tab.)  Include email contacts and which test is requested. (See “Titer Testing Details” tab for fees and tests offered).  Complete vaccination history as best you can.

3. Put serum sample and completed form with payment into separate plastic bags.  Include a check payable to University of Wisconsin (unless other arrangements made.)

4. Protect the sample from breakage – use bubble wrap or newspaper

5. Package and ship – USPS small box works well.  Print out and apply shipping label (see separate tab).
