CalfScan – Calf Lung Ultrasound Scorer

Now available on iTunes App Store.

The CalfScan app was developed at the University of Wisconsin School of Veterinary Medicine. The app uses a graphical interface to store and evaluate results from lung ultrasonography, as described by Dr. Terri Ollivett.

This app may be a useful tool when implementing the #WeanClean™ Philosophy which involves lung ultrasound scanning at four strategic points:

  • Weaning – how many have pneumonia at the start of weaning? Goal: < 15%
  • Start of treatment – how many score > 3 or < 2 at their first treatment? Goal: < 15%
  • 7-10 d after treatment – how many score > 2 after their first treatment? Goal: < 15%
  • 12×7 scans – starting at 7d of age, scan 12 at 7d intervals to find high-risk age group

.

Lung ultrasound scores are 0 – 5. Scores of 0 are considered normal. A score of 1 indicates possible interstitial disease, pulmonary edema, or early bacterial bronchopneumonia. A score of 2 indicates the presence of a lobular, or patchy, pneumonia that is likely bacterial in nature but may be viral if lesions are small. A score of 3 – 5 is indicative of a more severe lobar bacterial bronchopneumonia. Specifically, a score of 3 means that only one lobe is entirely consolidated, whereas a score of 4 means that two lobes are entirely consolidated, and a score of 5 means that three or more lobes are entirely consolidated.

In addition to the ultasound score, the lobes showing consolidation may be recorded by touching the lobe buttons labeled 1 to 7. The right side lung is represent by lobes 1 – 4 and the left side by lobes 5 – 7 as shown in the table below. Entering the calf’s birthdate will allow automatic calculation of the calf’s age. Comments and body weight may also be recorded. Save the record by touching the Save / New button.

The following table provides pictures and descriptions for each of the scoring categories.

 Ultrasonography

US = 0
(No or few comet-tails, less than 1cm consolidation, Normal)

US = 1
(Severe diffuse comet-tailing only, rarely occurs in dairy calves)

US = 2
(Patchy lesions totaling at least 1cm of consolidation, Lobular Pneumonia)

US = 3-5
(Lobar Pneumonia) 3 = 1 lobe, 4 = 2 lobes, 5 = 3 or more lobes consolidated

When the app is initiated, the user is prompted for a Herd Name and Scorer Name. This is to facilitate data storage and retrieval. For each calf scored an ID, body weight, ultrasound score, consolidated lobes and comment may be assigned. Save the record by touching the Save / New button. The new record will be added to the table view. Touching a record in the table view will select that record for editing.

To edit a saved record, touch the record in the data table view. Once editiing is complete, touch Save / New. If you wish to discontinue editing and start a new record, touch the selected record in the table view to deselect it. The data entry fields will be blanked out and ready for new data.

The Data Selection button (segmented button below the table view) allows the user to select all or subsets of the data stored. The Herd Today segment will display only data from the herd named in the Herd Name text field having a score date equal to the date displayed in the Date text field. The Herd All segment will display all of the data from the herd named in the Herd Name text field. The All Herds segment will display all of the data stored within the app.

There are 4 buttons at the bottom of the screen. The Delete button will facilitate the deletion of data based on the selected segment of the data selection button as explained above. The Data button will open a new window with a full screen table view of the selected data. The Compose button at the top left will open the email composer with an attached comma delimited file of the selected data. Simply enter an email address and press send to share the data. The History is similar to the Data button but will only display data selected with the Herd All button. The data will be sorted by Calf ID and Date to facilitate view calf score history. The Summary will provide summary information of the data selected by the Herd Today button.
